If different equipment is required, you will need to complete a full HOOF registering the patient at the new address. This needs to include section 7, 8 and 9 being completed as required.

A. No. Once the original installation has been carried out, the patient can call 0800 373580 to reorder cylinders without a new HOOF being completed.
